Emergency Care Form

Emergency Care Form

Current, complete and accurate parent/guardian contact information is the most important information you can provide to the school. The main purpose of the form is to help us locate the parent/ guardian in the event of illness or emergency involving your child. Hospitals and private doctors will not give emergency medication or treatment, or perform any kind of surgery except when the parent/ guardian is present.

It is very important that we are able to locate you at all times. Please list phone numbers of adults who will know where you are for emergency calls and who may pick up your child when he/she feels ill and you are not available. Make sure you list a phone number where you can be reached—not just voice mail. In special cases, you may wish to send a note or call and give the main office a number where you can be reached for the day. List on the form any allergies or serious medical problems.   Also, please list any special arrangements you may have for another person to pick up your child regularly, such as daycare. Children will only be released to those people listed on their emergency care card.

PLEASE COMPLETE THE EMERGENCY CARE FORM AND RETURN IT TO SCHOOL IMMEDIATELY.  REMEMBER TO UPDATE IT THROUGHOUT THE YEAR IF ANY INFORMATION CHANGES AND LET THE OFFICE KNOW.
